I release all such references from liability for any damage that may result from furnishing such evaluation to you, and I waive any right that I have to inspect the reference provided on my behalf.

Qualification & Conduct Agreement For All Living Word Church Ministry of Helps Volunteers

Christians who are in places of responsibility in the church are required to be examples in faith, conduct, and business affairs keeping in mind that you are representing the Lord, Living Word Church and its members. To maintain a high standard for workers is one of the best ways to present Christ to the people of our community. Therefore, the following guidelines will be required of any person who works in any of the ministries of Living Word Church.

  • Must be born again.
  • Must live a life separated unto God.
  • Must remain sexually pure including abstaining from pornography, fornication, adultery and homosexuality.
  • Must be an active member of Living Word Church, being faithful in attendance and tithing.
  • Must be in agreement with the tenets of faith of LWC.
  • As part of the ministry of helps, I agree not to put anything on Facebook that would damage my testimony or the testimony of any church member or the ministry.
  • Be loyal to the Pastors and Leaders of LWC.
  • Complete an application, and be approved by the Department Head and Pastor Olson.
  • Dress neatly and be well-groomed.
  • Be at your designated post thirty (30) minutes before starting time.
  • Attend all training meetings.
  • Must be teachable, submitted to authority, and willing to follow instructions.
  • Give at least three days notice or get replacement if you are scheduled to work in a certain area but will be absent.
  • Give thirty (30) days notice when resigning your position.

 For the safety and well-being of our congregation, all individuals seeking to serve in ministries involving children or church security are required to undergo a background check.

NOTICE – BACKGROUND INVESTIGATION

In connection with your employment with Living Word Church of Stone Co., Inc (the “Company”), notice is hereby given that a consumer report and/or investigative consumer report may be obtained from a consumer reporting agency for employment purposes. These reports may contain information about your character, general reputation, personal characteristics and mode of living, whichever are applicable. They may involve personal interviews with sources such as your neighbors, friends or associates. The reports may also contain information about you relating to your criminal history, credit history, driving and/or motor vehicle records, education or employment history, or other background checks.

You have the right, upon written request made within a reasonable time after the receipt of this notice, to request disclosure of the nature and scope of any investigative consumer report prepared by contacting the Company and Protect My Ministry 14499 N. Dale Mabry Hwy., Suite 201 South, Tampa, FL 33618; Phone: 1-800-319-5581. For information about Protect My Ministry’s privacy practices, see www.protectmyministry.com. The scope of this notice and below authorization is not limited to the present and, if you are hired, will continue throughout the course of your employment and allow the Company to conduct future screenings for retention, promotion or reassignment, as permitted by law and unless revoked by you in writing.

ACKNOWLEDGEMENT AND AUTHORIZATION

By providing my digital signature below I hereby authorize the obtaining of consumer reports and/or investigative consumer reports by the Company at any time after receipt of this authorization and throughout the course of my employment, if applicable.
